A 39-year-old man sustained facial lacerations from the body of a roe deer (Capreolus capreolus) and shattered glass after the animal, first struck by an oncoming vehicle, broke through his windshield. Animal blood, bodily fluids from eviscerated organs (eg, intestines), and soil contaminated his wounds (figure A). 8 days later, subjective fever, a left periorbital lesion, and ipsilateral cervical swelling developed. On day 12, sonography found cervical lymphadenitis without abscess formation. Despite amoxicillin treatment (1 g three times a day for a week), the swelling progressed (figure B).
