A 90-year-old, bedbound woman was admitted with low-grade fever and a painful, foul-smelling, exophytic mass in the right preauricular region. Basal cell carcinoma had been diagnosed a year earlier, but treatment was delayed due to non-attendance at follow-up. On examination, the tumour had devitalised tissue and visible maggots within the wound (figure A).
